It can be :in the form of metes d bounds survey or a des~rip- <br />tion by reference to township, rouge, and section, [o at least the neatest quarter- <br />quarter section. Wht:re applicable, tlhe street: address or lot number(s) my be used. <br />EXHIBIT B - Mining Plan <br />This eaa be a genera]. discussion of the methods, equipment, phases or stages of the <br />operation and those portions of land affected by each. The plan should be correlated <br />with the map and .should show where extraction will begin, and which direction, if any, <br />it will proceed. <br />~CIIIBTT C - Man <br />This map should clearly define permit boundaries and indicate all roads or stockpile <br />areas to be used in connection with the operation. All imaediacely adjacent landowers <br />must be show. The quarter section comer, section line, or any other locate able <br />points should also be show-on this map.- - `" <br />EXHIBIT D - Wildl Sfe., Rater Resources. Vegetation and Soils Information <br />Each of the above-mentioned subjects must be addressed as outlined in [he applicable ~- <br />RULE. - - <br />EXHIBIT E - Reelamat:Con Plan <br />As with the mining plan, this should be a general discussion of the methods, equipment, <br />phases or stages of the reclamation process and those portions of land affected by each. <br />If possible, the plan should be closely correlated vi th the map. If not, Che narrative r <br />must be comprehensive enough to draw an accurate verbal picture of the expec red appear- <br />ance of the area after completion of reclamation. _, <br />EK9IBIT F - Proof of Local Government Approval <br />This must be a letter from the local government specifying conformance with existing <br />zoning regulations. (I[ is intended that operators may obtain local gone r..ment approval <br />concurrent wi ch tie i]ivision's processing of the application. To do so, the operator - <br />must submit evidence which indicates Chat local government approval is currently pending <br />or sill at least be considered within a reasonably short time by the appropriate govern- <br />ing body RULE L.1(10) (Consult this RULE for full details on this requirement.) <br />~'HI3IT G - Source of Leeal Rieht to Enter <br />This could be a copy of the lease, deed, abstract of title, or current tax receipt. _ <br />An acceptable alternative would be a statement acknovled gad by a notary public [hat <br />the operator has the legal right ~to eater and mine. <br />EXEiIBIT A - Reclamation Costs <br />For a 110 permit, if the operator requests surety to be less than $2500, the ooera[or vrsst~ <br />submit facts to support such a recuest. Otherwise, a surety of $2500 will automatically <br />be set. For all 111 permits, the surety shall be $2500 per acre. <br />EXHIBIT I - Terms of Gove rnmen[al Contracts - <br />For Special 10-Day (111) Perr3ts, the operator must submit a copy of the contract or <br />other proof that prcves the necessity of a special l0 may permit. The operator must <br />also submit evidence of the surety provided under the govern~eatal contract, i-f it vas <br />required. <br />NOTICE PROCEDURES <br />After the applicaticn is considered "filed" (pursuant to RLZE 1.1(10) with the `tined <br />Land Reclamation Div *_sion, notice procedures outlined in RULES 3.2 and/or 4.2 must be <br />followed.
