To this date, we have no <br />record of having received the annual report as requested. Submission is <br />required by the Colorado Mined Land Reclamation Act, Article 32 of Title <br />34, C.R.S. 1973, as amended and by the Rules and Regulations promulgated <br />thereunder. Your attention is directed to the following provisions f-om <br />Rule 2.4 for operators holding a regular, 112 pennit and Rule 3.4 for <br />operators holding a limited impact, 110 permit. <br />2.4 Annual Reports <br />2.41 Un the anniversary date of each permit year, the operator shall <br />submit a report and map (to a scale of not less than 100' to the <br />inch and nut more than 660' to the inch}, indicating the pha<•es <br />of the reclamation plan which have been completed, correlated <br />with the timetable. The report and rnap shall contain the <br />following information: <br />(1) Location and number of acres affected during that year, <br />(2) Locations and number of acres reclaimed for that year, <br />(3) The location and nwnber of acres in various stages of <br />reclamation (backfilled, yraded, topsoil replaced, <br />reveyetated), <br />(4) The species of vegetation planted, the locations and the <br />approximate dates of planting, <br />(5) The type and approximate quantity of fertilizers, organic <br />material or soil conditioners used and location of use, <br />(6) An appraisal of the success of reclamation efforts, and in <br />the event of failure of any efforts, a short narrative on <br />the suspected or determined causes of such failure, <br />(7) Estimates of the location and nwnber of acres to be <br />affected in the cominy year, and <br />(8) The effective date of the information provided above shall <br />be no snore than 45 days prior to the anniversary date. <br />~~ <br />423 Centennial Building, 1313 Sherman Street Denver, Colorado 80203 Tel.
