Two prints of revised Exhibit E, Mining and Reclamation Plan. <br />2. Two copies of the Performance and Payment Bond to he included as part of <br />Exhibit K. <br />3. An original revised Financial Warranty in the amount of $55.000.00. <br />We have also included an Index Map to assist you with your adequacy review of our <br />application. The following items have been added to Exhibit E and are highlighted on the <br />Index Map: <br />Names of owners of record of the affected surface area and within 200 feet of the <br />the permit boundary. <br />A list of owners of man-made structures within 200 feet of the affected area. <br />The owner of the substance to be mined. <br />The direction of mining. <br />The average thickness of replaced topsoil. <br />
