Name, address, and telephone number of person seeking to explore / <br /> <br />3. Name, address, and telephone number of [he representative who <br /> will be present at and responsible for conducting the exploration <br /> activities (2.02.2(2)(b)). <br />4. Map (1:24000 or larger) and narrative description of the exploration / <br /> area (2.02.2(2)(c)). <br />5. A statement of the period of intended exploration (2.02.2(2)(d)). ,/ <br />6. If the surface is owned by a person other than the person who <br /> intends to explore, a description of the basis upon which the person <br /> who will explore claims the right to enter such area for the purpose <br /> of conducting exploration and reclamation (2.02.2(2)(e)). <br />7. A narrative description of the practices proposed to be followed to <br /> protect the environment from adverse impacts as a result of the / <br />~/ <br /> exploration activities (2.02.2(2)(f)). <br />8. A narrative description of the methods [o be used to conduct coal <br /> exploration and reclamation, including, but not limited to, the types <br /> and uses of equipment, drilling, surface blasting, road or other <br /> access road construction (4.21.4(3)), excavated earth or other debris <br /> disposal activities, and other information set forth in Rule <br /> 1.04(127). The description shall set forth the maximum number of <br /> drill holes the person conducting the exploration intends to drill <br />'~ <br /> within each quarter-quarter section (2.02.2(2)(8)). <br />9. A statement, with appropriate references from [he relevant State or <br /> Federal agencies or published sources, which determine that the <br /> exploration and reclamation described will not jeopardize the <br />continued existence of an endangered or threatened species listed <br /> pursuant to Section 4 of the Endangered Species Act of 1973 <br /> (U.S.C. 1533) or the Nongame, Endangered or Threatened Species <br /> Conservation Act (Section 33-8-101 et sea• C.R.S.) or result in the <br /> destruction or adverse modification of critical habitat of those <br /> species (2.02.2(2)(h)). <br />M 9oss\jrt\NOICHECK.ISI' <br />
