Upon penny issuance and prior to begvming on-site mining activity, you must poll a sign at the enUance of the <br />mute site, which shall be clearly visible from the access road, with the following information (Rule 3.1.12): <br />a. the name of the operator; <br />b. a statement that a reclamation pemtit fa the operation has been issued by the Colorado Mined Land <br />Reclamation Board; an4 <br />c. the permit number. <br />7. For ioint venture/oarmership opcrators: the signing representative is authorial to sign this doaartatt and a power <br />of attomty (provided by the partner(s)) authorizing the signature of the reprcsrntative is attached to this application. <br />Permit Condition: <br />A reportable spill is a spill of arty toxic or hazardous substance (irtcltrding spills ofparoletmr produRC) within the <br />milted land permit area reportable to arty Division of the Colorado Departmem of Public Health and the Environment, the <br />J National Response Center, the Cobrado E~rgeocy Plamvng Comrnissiaa azry local Emergency Planning Commissiaq <br />local Emergency Planning Committee, or the Slate Oi] Inspector. The Mme Operator shall notify the Dnision of Minerals <br />and Geology (DMG) of a reportable spill within the mined land permit area using the same timeframe required by the <br />permit, license, notice, statute, tide or regulation governing the reporting of the spill to the other appropriate agency. <br />Notice of a reportable spill shalt be FAXed to: Minerals Program SupenTSOr, I?iv~sion of Minerals and Geology, <br />FAX(303)832-8106.
