Colorado 80203 <br />(303) 289-8555 Fax: (303) 289-7707 <br />RE: Submittal of DMG 112 Permit Application for Nelson Mining Resource ("NOI" <br />Permit No. P-2005-011) <br />Dear Suzi: <br />Enclosed, please find a complete submittal for the referenced application for sand and <br />gravel mining on the Nelson Mining Resource site. The site has a recently approved <br />"NOI" for it with a permit number of P-2005-01 I. As with the Turnpike Mining <br />Resource amendment, I am assuming that you will be the one to review the application <br />for completeness. This is a very straightforwazd permit application and mining site. The <br />submittal includes: <br />• Original application with signatures in blue ink, initialed, and notarized and a <br />copy of same. <br />• A check in the amount of $2,344.00 made out to the Division of Minerals and <br />Geology to wver the cost of the permit application review. <br />• Proof of faxing on today's date of Notices to the Weld County Commissioners <br />and the Longmont Soil Conservation District by a signed affidavit from each <br />entity. <br />• Affidavit of the Weld County Clerk to the Boazd that a copy of the application <br />submittal was placed for public inspection on today's date at the Weld County <br />Clerk to the Board's office. <br />• Certificate of Posting showing that the property was posted on 7-21-05. <br />• Two unbound copies of the submittal document. <br />
