Dear Mr. Peterson:
The Division completed a technical review of your 112 Regular permit application. Following
aze items that need to be addressed so the application will meet the minimum requirements of the
Act/Mineral Rules and Regulations:
1. Exhibit A - OK
2. Exhibit B - OK
3. Exhibit C -The maps have not been signed pursuant to requirements of Rule 6.2.1(2)(b).
Please have the maps signed by the person who prepazed them and submit 2 copies.
The pre-mining map does not describe the existing vegetation on the proposed 320-acre
permit azea. Please provide a description of present day vegetative cover.
The pre-mining or mining plan maps do not show the existing topography. However,
Exhibit B does provide the required information.

6. Exhibit F - It appears the submitted reclamation plan map shows existing topography, not
expected future topography. Is this correct?
Also, the map does not have a map scale, no north arrow, no applicant's name, no
signature from the person who prepared the map, no prepazation date, and no permit
boundary, as required by Rule 6.2. Please provide all the required information and submit
2 copies to the Division.
7. Exhibit G - Pazagraph 2(b) you need to finish the sentence.

10. Exhibit J -You need to submit an inventory of the existing vegetation. The document you
submitted contains the proposed seed mix.
