' -5- <br /> ' EXHIBITS A-J <br /> Two (2) complete application packages must be submitted. One complete application <br /> package consists of the application form and the set of maps and exhibits referenced <br /> ' below as Exhbits A-J. Each exhibit must be presented as a separate section. Begin each <br /> exhibit on a new page. Pages should be numbered consecutively for ease of reference. <br /> If separate bound documents are used as appendices, please reference these by name in <br /> the exhibit. <br /> EXHIBIT A - Legal Description and Location Map <br /> ' The legal description must identify the affected land, specify affected areas and be <br /> adequate to field locate the property. Description shall be by township, range, and <br /> section, to at least the nearest quarter-quarter section. A metes and bounds survey <br /> ' description is also acceptable. Also, the street address or lot number(s) may be used. <br /> This information is available from the County Assessor' s office or U.S. Geological <br /> Survey (USGS) maps. NOTE: If you improperly locate your operation in the field, and as <br /> a result mine outside the area delineated by this permit application, it could be <br /> considered to be a violation of 34-32-123 or 34-32-124, C.R.S. of the Mined Land <br /> Reclamation Act. <br /> ' . A map showing information sufficient to determine the location of the affected land on <br /> the ground and existing and proposed roads or access routes to be used in connection <br /> with the mining operation. Names of all immediately adjacent surface owners of record <br /> ' shall also be shown. The operation location map shall be a standard 1 :24,000 scale U.S. <br /> Geological Survey map. The location of the proposed operation shall be shown and <br /> labeled with the mine site name. <br /> ' EXHIBIT B - Site Description <br /> Items 1-3 below must be addressed to the extent necessary to demonstrate compliance with <br /> tthe applicable performance standard requirements of Rule 6. At a minimum, you should <br /> include the following information: <br /> ' 1 . A description of the vegetation and soil characteristics in the area of the <br /> proposed operation. The local office of the Soil Conservation Service (SCS) may <br /> provide you with this information as well as recommendations for Exhibit D - <br /> ' Reclamation Plan. Contact the Regional SCS office at 655 Parfet Rm E200C, <br /> Lakewood, Colorado 80215, (303) 236-2886 or the Mined Land Reclamation Division <br /> for assistance in locating the office nearest you; <br /> ' 2. Identify any permanent man-made structures within 200 feet of the affected <br /> area and the owner of each structure. Permanent man-made structures include <br /> houses , barns , fences, above or below ground utilities , irrigation ditches , <br /> ' maintained or public roads , bridges, wells , pipelines, water storage impoundments , <br /> railroad tracks, cemeteries , and communication antennas , etc. Each structure <br /> should be located on Exhibit E - Map. <br /> ' 3. A description of the water resources in the area of the proposed operation. <br /> Identify any streams , springs, lakes, stockwater ponds , ditches , reservoirs , and <br /> aquifers which would receive drainage directly from the affected area. Provide any <br /> ' information available from publications or monitoring data on flow rates, water <br /> table elevations and water quality conditions . <br />
