The access shall be constructed, maintained and used in accordance with the terms and conditions of this permit,
including the State Highway Access Cotle and listed attachments. This permit may be revoked by the Issuing
authority it at any time the permittetl access antl its use violate any of the terms antl conditions of this permit. The use
of advance warning antl construction signs, Ilashers, barricades and flaggers are required at all limes during access
construction within Stale right-of-way in conformance with the MANUAL ON UNIFORM TRAFFIC CONTROL
DEVICES, Part VI. The issuing authority, the Deparlmenl and theirtluly appointedogents and employees shell be held
harmless against any action for personal injury or properly damage suslainetl by reason of the exercise of the permit.
