Preliminary review of the information received <br />determined that the following items must be received before the Division can consider your application as being <br />submitted and technical review can begin: <br />APPt.t(`ATinN FnRM~ The 112 Construction Materials application form received by the Division ou <br />June 25, 2004 is no longer an application form approved by the Mined Land Reclamation Board. An approved <br />form is enclosed for your use. Item No. 3 on the application form received on June 25, 2004 was not completed. <br />You must provide the source of the <br />Opentor'slApplicarn's legal right to enter and initiate a mining operation on the affected land (Same <br />requirements as Rule 6.3.7). You may provide a copy of Farfnunwurkin, LLLP's deed, abstract of title, or a <br />correct. tax receipt. Anyone wishing to comment on the application may <br />view the application at the County Clerk's or Recorder's office and should set3d comments prior to the end of the <br />public comment period to the Colorado Mined Land Reclamation Office, at the address given on the cover of <br />these Rules and Regulations. Any class of Limited Impact 110 or 110d operation need only post notice (sign) at <br />the location of the proposed access to the site. After having posted such notice (sign), failure by an Apphcaat to <br />maintain such notice shall not constiiute just cause to deny approval of the application.
