Please provide to scale a visible, locatable, and permanent reference <br />point on the map. Please provide the names of all owners of record of the surface of the <br />affected area and of the land immediately adjacent to the affected area. <br />Financial Warranty <br />The maximum amount of financial warranty for a Limited Impact Operation is <br />55,000.00. If you wish to commit to a E5, 000.00 financial warranty, then you <br />do not need to provide an estimate of reclamation costs. If you want to post <br />a uncial warranty in a lesser amount, then you must provide an estimate of <br />the actual costs to reclaim the site based on what it would cost an <br />independent contractor to complete reclamation. This should include unit <br />costs for the following activities as appropriate to the operation: <br />backfilling, grading, topsoil application, seeding, mulching, fertilization, <br />and labor. <br />Thank you for contracting our office.
