<br />2. Has the Applicant provided an appropriate blast vibration analysis and blasting plan <br />demonstrating that off-site azeas will not be adversely affected by biasing? At issue are <br />potential damages to: the Black Hawk/Central City Sanitation District's Waste Water <br />'Treatment Plant, sewage lines to the Cleaz Creek Convenience Store (owned by Cleaz <br />Creek Convience LLC) San, Silver Dollar Metro District light poles, and utility lines <br />along Colorado Highway 119 (Rules 6.4.4(1) and 6.5(4) ) <br />Has the Applicant identified all significant, valuable, and permanent man-made structures <br />within 200 feet of the proposed affected land? (Rule 6.4.19 -Exhibit S) <br />4. Has the Applicant accurately identified and located on a map all surface and groundwater <br />resources on the affected land and adjacent lands that may be affected by the proposed <br />mining operation? (Rules 6.4.7 - Exhibit G and 6.4.3(f) -Exhibit C) <br />5. Has the Applicant submitted a brief statement or plan showing how water from de- <br />watering operations or runoff from the disturbed areas, piled material and operating <br />surfaces will be managed to protect against pollution of surface water (and, if applicable, <br />control pollution in a manner that is consistent with a water quality dischazge permit), <br />both during and after the operation? Specifically, are potential impacts to North Cleaz <br />Creek identified? (Rule 6.4.7(2)(c)) <br />6. Has the Applicant provided an adequate assessment of the disturbances to the prevailing <br />hydrologic balance of the affected land and of the surrounding area and to the quantity <br />and quality of water in surface and ground systems? (Rule 3.16). Specific issues are: <br />• potential for contaminants to enter the interconnected fractures supplying the <br />- _- - .--- - - ---- --water to the neatbyweils impaetingwaterquaiity~ ------- <br />• potential for reduced yield from the wells; <br />• potential for uranium mineralization to enter ground water via blasting and <br />mining activities; and <br />• an adequate monitoring program to ensure the protection of surface and <br />groundwater resources. <br />Has the Applicant demonstrated compliance with applicable Colorado water laws and <br />regulations governing injury to existing water rights? (Rule 3.1.6(1)) <br />8. Has the Applicant stated how it will comply with applicable groundwater performance <br />standazds? (Rule 3.1.7) <br />9. Has the Applicant adequately described the game and non-game resources, including <br />threatened and endangered species, on and in the vicinity of the application area, and <br />provided a description of the impacts to such resources during mining and reclamation, <br />and appropriate mitigation measures? (Rule 6.4.8 -Exhibit H) <br />
