EMERGENCY RESPONSE PLAN WHIRLWIND MINE <br />• PROCEDURE NO. WW-1 Revision No. New Replaces Procedure: N/A <br />Page: 7 of 11 Revision Date: New Dated: 06/29/07 <br />Sounding of the Emergency Alarm Hom (Fire Alarm) during the Notification Phase of an emergency <br />situation constitutes the Alert phase of a Stage 2 response operation and signals the beginning of a <br />Stage 2 site response. All EFR and non-EFR personnel on the surface areto immediately proceed <br />to a designated assembly point when the Emergency Alarm Horn (Fire Alarm) is sounded. <br />2.2.2 Stage 2 Mobilization <br />All emergency responses to an emergency situation at the Whirlwind Mine will continue through <br />Stage 2 Mobilization. Mobilization for Stage 2 response operations consists of the assembly and <br />orgartlzatton of site surface personnel for coordinated response operations and will terminate at the <br />end of mobilization if the emergency situation is successfully resolved at the First Responder level. <br />The site will demobil'~ze and the Remediation Phase of the Emergency Response Plan will be <br />executed if the situation is resolved at the First Responder level. <br />Mobilization: Assembly <br />All surface personnel are required to reportto a designated assembly pointwhen the alert signal is <br />sounded. The primary assembly point for EFR personnel during a surtace emergency situation is <br />the Fire Alarm Control Panel located at the northeast corner of the Maintenance Building. The <br />• primary assembly point for non-EFR personnel is the East End of the Waste Dump. The Main <br />Office Trailer is the alternative assembly area if the primary assembly points are inaccessible. <br />Mobilization: Basic PPE Requirements <br />All personnel reporting for Stage 2 response operations will first don hard hats, safety glasses, and <br />steel-toed safety shoes. This is the minimum personnel protective equipment (PPE) required <br />during response operations. EFR personnel who are not wearing the minimum PPE listed above <br />will be restricted to support functions only during Stage 2 operations. <br />Mobilization: Mine Forman (Emergency Coordinator - EC) <br />^ Provide an initial situation report to all response personnel at the primary assembly point. <br />Review the incident evaluation, determine a preliminary course of action, and delegate <br />specific duties to response personnel. <br />^ Establish an incident command post at a secure location near the incident location. <br />Establish a ails command post with secure communication (inks to off-site entities at a <br />location unaffected by the incident and ensure that communications with the incident <br />command post and off-site entities are functional. <br />^ Organize response personnel for deployment based upon the initial situation report and the <br />projected course of action. Response organization will include the delineation of <br />responsibilities or duties; the provision of proper PPE; the collection of speGalized <br />equipment; and the establishment of a support base, including communications. <br />• Establish a preliminary field response plan based upon the initial situation report and the <br />projected course of action. <br />ENERGY FUELS RESOURCES HEALTH & SAFETY PROCEDURES <br />
