ComRliance Information <br />• Eastside Coal Company Inc. ,any subsidiary, affiliate, or persons controlled by or under the <br />common control with Eastside Coal Company, Inc. has: <br />Never had a Federal or State mining permit for surface coal mining operations suspended <br />or revoked in the last five years prior to the date of submission of this application; or <br />Never forfeited a mining bond or similaz security deposited in lieu of bond. <br />Eastside Coal Company, Inc. has not received any violation notices in connection with any surface <br />coal mining operation during the three year period before the date of this application. <br />Eastside Coal Company, Inc. has received the following notices of violation: <br />Nov 83-20 -issued November 8, 1983 by Colorado Mined Land Reclamation Division. <br />Diversion Ditch not completed, improper storage of waste material, and sediment <br />pond dewatering device not installed -Assessment conference penalty accepted. <br />Nov 83-34 -issued December 15, 1983 by Colorado Mined Land Reclamation Division. <br />. Selling coal without Colorado Mined Land Reclamation Division approval -- <br />Informal hearing held at the minesite -- Assessment conference penalty paid. <br />Nov C-93-149 -issued December 15, 1993 by Colorado Mined Land Reclamation <br />Division . <br />Failure to provide notification of the approved mining schedule to all owners of <br />property and residents within the area above the underground workings and <br />adjacent areas. <br />Nov C-96-014 -issued June 21, 1996 by Colorado Mined Land Reclamation Division. <br />Failure to submit Annual Reclamation Report and failure to make mine records <br />immediately available to the public.
