Long <br />Divwon Dveclor <br />Re: Indian Springs Park Pit, File No. M-2001-087, Receipt of 112 Construction Materials Reclamation <br />Permit Application Package <br />Dear Mr. Klco: <br />On August 16, 2001, the Division of Minerals and Geology received your I12 Construction Materials <br />Reclamation Permit Application package for the Indian Springs Park Pit, File No. M-2001-087. Preliminary <br />review of the information received determined that the following items must be received before the Division can <br />consider your application as being submitted and technical review can begin: <br />APPLICATION FEE: The Division did oat receive the correct application fee of $2,344.00. All <br />construction material fees increased on July I, 2000. Please remit an additional $469.00. / <br />APPLICATION FORM: The Division will accept the application form, received on August 16, 2001, <br />even though it is not the current approved application by the Mined Land Reclamation Board. 't'he <br />Division canna[ accept this form in the future. We have enclosed new fotms (t 10, 111, and 112 forms) <br />for your use. You will need to contact the Division when you get close to nnrttutg out of the proper fomrs <br />so that additional forms can be sent to you to the future. <br />EXRIBIT N -Source of Legal Rieht to Enter (Rule 6.4.141: You must provide the source of your <br />legal right to enter and initiate a mining operation on the affected land. The letter from the State of <br />Colorado Board of Land Commissioners does not specify the lands that they will grant the Fremont <br />County Board of Commissioners a sand and gravel permit. Please submit documentation from the <br />Colorado Board of Land Commissioners which identify the land which will be coveted in Sand and <br />Gravel Permit GP 3437. <br />Your application will not be considered submitted until the information listed above is received and found <br />sufficient to begin our review. A decision date will be established 90 days from the date of receipt of all of are <br />requested information. Additionally, if you have already published notice you will need to republish notice, after <br />the Division considers the application submitted. This notice must be published once a week far four (4) <br />consecutive weeks, starting within ten (10) days of the date your application is considered submitted. We will <br />notify you when you should initiate republication of your notice.
