Box 338 Cotopaxi, CO 81223 Re: Indian Springs Park Pit, File No. M-2001-087, Receipt of 112 Construction Materials Reclamation Permit Application Package Deaz Mr. Klco: On August 16, 2001, the Division of Minerals and Geology received your 112 Construction Materials Reclamation Permit Application package for the Indian Springs Park Pit, File No. M-2001-087. Preliminary review of the information received determined that the following items must be received before the Division can consider yow application as being submitted and technical review can begin: APPLICATION FEE: The Division did not receive the correct application fee of $2,344.00. All construction material fees increased on July 1, 2000. Please remit an additional $469.00. APPLICATION FORM: The Division will accept the application form, received on August 16, 2001, even though it is rat the current approved application by the Miued Land Reclamation Board. The Division cannot accept this form in the future. We have enclosed new forms (110, 111, and 1 ]2 forms) for yow use. You will need to contact the Division when you get close to running out of the proper forms so that additional fom3s can be sent to you in the firture. EXffiBIT N -Source of Lesal Rieht to Eater (Rule 6.4.14): You must provide the sowce of yow legal right to enter and initiate a mining operation on the affected land. The letter from the State of Colorado Board of Land Commissioners does rat specify the lands that they will grant the Fremont County Board of Commissioners a Saud and gravel permit. Please submit documentation from the Colorado Board of Land Commissioners which identify the land which will be covered in Sand and Gravel Permit GP 3437. Yow application will not be considered submitted anti the information listed above is received and found sufficient to begin our review. A decision date will be established 90 days from the date of receipt of all of the requested information. Additionally, if you have already published notice you will need to republish notice, after the Division considers the application submitted. This notice must be published once a week for four (4) consecutive weeks, starting within ten (10) days of the date your application is considered submitted. We will notify you when you should initiate republication of yow notice. The final date for receiving comments is the 20th day after the fourth publication or the next regulaz business day.
