Box 597 <br />Y.iowa, CO 80117 <br />Re: File No.: M-88-101, Fondis Pit, Incomplete Application <br />Dear Mr. Ullom: <br />Upon reviewing your application fora Mining & Reclamation Permit, received <br />October 6, 1988, we have determined that it is incomplete under the Colorado <br />Mined Land Reclamation Act (C.R.S. 34-32-101, et ~.), and the associated <br />regulations. <br />The following items must be submitted in order for the application to be <br />considered complete (Rule 1.1(10) of the Rules and Regulations of the Mined <br />Land Reclamation Board, as amended, effective.July, 1978): <br />1. The last page of the Application Form with an original signature of <br />the Applicant. <br />2. An additional copy of the entire Application, including all <br />Exhibits thereto. <br />3. Exhibit B, Mining Plan and Exhibit C, Map as described in detail in <br />the Limited Impact Operation (110(2)) Application Package, and such <br />other information as may be required to meet the requirements of <br />paragraph 3.11 under Rule 3 of the Mineral Rules and Regulations. <br />Specifically, please address the location of stockpiles, include <br />sufficient topographic data and explanatory text to show the <br />relationship between the proposed mine site and the adjacent stream <br />channel, and identify any permanent, man-made structures within 200 <br />feet of the permit boundary. <br />+ , <br />We would also appreciate it if, in the future, you would use our most recent <br />application form, a copy of which is enclosed for your reference. This form <br />includes a description of the information needed to satisfy the deficiencies <br />noted above.
