6. Finally, please ensure that any easements which are noted in Application Item No. 7.1 are also depicted and
identified on the map.
7. The existing vegetation on the site must be depicted, especially since it may involve wetlands.
8. The existing ditch onsite is shown to be realigned during the mining phase, though parts of the new location are
outside the affected area. The old and new ditch alignments are shown to exit the Gately property at different
locations, raising the question of the adequacy of the realignment or of any other offsite work will be needed to
ensure that the ditch functions. (For example, the southwest comer of the new alignment of the ditch is up the hill,
and not on the level haymeadow. If this is not a pipeline siphon system, the new route shown will not be feasible.)
You must ensure that all areas affected by all aspects of the operation, such as equipment parking, processing and
stockpiling areas, ditches, internal roadways, dewatering locations and areas to be graded as pond slopes, are
included within the boundary of the affected area. Please be reminded that details in the mining plan (Exhibit D)
must be shown as features on this map.
9. Please show the direction of mining, either by use of arrows or lines delineating phases and/or by use of more
than one map. Please show all topsoil stockpile locations.
Correcting these inadequacies will require a map revision. If there are questions please call to ensure that the
required changes are incorporated in the revised map.
Exhibit D -Mining Plan (Rule 6.4.4)
10. Clarifications to the following items should consist of a narrative explanation in this exhibit and, when
appropriate, depiction of them on the mining map. Please be as specific as possible, since these responses will
affect the bond amount.
11, You state that a perimeter fence is to be constructed. Since the mine site is divided into two parts by the old
railroad grade, will there be two perimeter fences or just one? Will the fence completely encircle the entire permit
area, or only part or the area, or part of the perimeter? Will there be any gates or cattleguards?
12. Is the old railroad grade shown on the maps the same as the "road" which is named in this exhibit? Is this the
road to be used as primary access to the site for this operation?
13. The operation is to consist of a series of phases, which will affect the different pit azeas shown on the map. At
what point of the operation will the Griffen # 1 Ditch be rerouted?
14. You mention dewatering in Exhibit G, but a good description of that aspect of your mining plan must be
included here. Please include where the pump(s) will be, and where the discharge will be directed.
I5. Please indicate where the wash plant will be located and which portion of the pond will be used for recycling
the wash water. Do you expect that silt will be dredged and placed elsewhere for any reason?
16. Please include better description of the equipment to be used for excavation, grading the slopes, topsoil
replacement, processing material, and hatching on the site.
17. What is the plan for disposal of crusher fines and/or waste asphalt? Please be aware that onsite disposal
(burial) of waste asphalt may not be appropriate with the shallow depth to groundwater.
18. Please commit to the adequate secondary containment of all hydrocarbons (fuel, Tube, hydraulic fluid, etc.)
which may be stored onsite. Please describe the location and the method of such secondary containment.
