A NOTICE TO PROCEED TO CONSTRUCTION, CDOT Form 1265, is required before <br />beginning the construction of the access or any activity in the highway right-of--way. The <br />Notice to Proceed to Construction will be issued upon receipt of this signed permit with <br />appropriate fee. <br />z. The access is located on the north side of State Highway SoA, at County Road 69 or <br />approximately milepost 274.3• <br />3. This section of highway is a Category R-A highway. <br />4. This TEMPORARY Access Permit is issued for a period of 1z months and will expire <br />August t, zoos to allow access to State Highway So for a change in use of the properh,~. <br />The previous use of the access was to serve a county road and several single-family <br />residences. The access will now serve a county road, several single-family residences and <br />a gravel quarry. This permit is issued for Phase I of this development. Significant <br />roadway improvements will be required upon the issuance of an access permit for Phase <br />II. <br />5. No new construction or improvements are required by the issuance of this Access Permit. <br />6. The Permittee shall refer to all additional standard requirements attached to this permit. <br />This includes CDOT Form totb, enclosed additional terms, conditions, exhibits, and <br />noted attachments. <br />• 7• The following criteria were used to establish this Access Permit: <br />a. The Application for Access Permit (CDOT Form 137) dated April 24, 2006 and <br />received in the regional office on April zs, 2006 and all attachments. <br />b. State Highway Access Code, Volume 2, CCR-Got-t; Effective date August 31,1998 <br />c. The State Highway Access Category Assignment Schedule, as revised. <br />d. The Colorado Department of Transportation (CDOT) M&S Standard Plans <br />e. Vicinity Map <br />8. All discharges to the CDOT highway drainage system must comply with the applicable <br />provisions of the Colorado Water Quality Control Act and the Colorado Discharge Permit <br />Regulations, and are subject to inspection by the CDOT and CDPHE. Although (City or <br />County Name) may or may not be a municipality that is required to obtain MS-4 permits, <br />CDOT would like to emphasize the this development devise and implement a permanent <br />plan for periodic removal and disposal of sediment from detention facilities and for <br />maintenance of development detention facilities. Attached is the CDOT Environmental <br />Clearances Information Summary listing some of the more commonly encountered <br />environmental permits/clearances that may apply to activities and contacts for questions <br />regarding these permits/clearances. <br />This Access Permit is issued in accordance with the 1998 State Highway Access Code <br />(zCCR Got-t), and is based in part upon the information submitted by the Permittee. <br />This Access Permit is only for the use and purpose stated in the Application and on the <br />Permit. Any changes, based upon existing and/or anticipated future conditions in traffic <br />volumes, drainage, types of traffic, or other operational aspects may render this permit <br />void, requiring a new Application for Access Permit to be submitted for review by the <br />Department and/or Issuing Authority. <br />u <br />
