Footnotes (continued) <br />g/ - Procedure for determining settleable solids is contained in 40 <br />CFR 434.64. The method detection limit for measuring settleable <br />solids under this part shall be 0.4 ml/I. <br />- A new source is generally defined as a coal mine (or major <br />alteration to a coal mine) the construction of vhich commenced <br />after May 4, 1984. See 40 CFR 434.11(j) for the complete <br />definition. <br />C. REPORTING REQUIREMENTS <br />1. Reporting <br />Monitoring results obtained during the previous 3 months shall be <br />summarized and reported on Division approved discharge monitoring <br />report forms, postmarked no lacer than the 28th day of the month <br />following the completed calendar quarter. For example, for the 1st <br />quarter (January, February, and March) the quarterly report must be • <br />received at this office postmarked no later than April 28. If no <br />discharge occurs during the reporting period, "No Discharge" shall be <br />reported. <br />The first original copy of each discharge monitoring report (DMR) <br />shall be submitted to the Division at the following address: <br />Colorado Department of Health <br />Water Quality Control Division <br />Permits and Enforcement Section <br />4210 East 11th Avenue <br />Denver, Colorado 80220 <br />The first duplicate signed copy of each report Form shall be submitted <br />to the following agency: <br />U.S. Environmental Protection Agency <br />Water Management Division <br />NPDES Branch 8WM-C <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />• <br />
