QOLOj%.-jO DEPARTME39OF PUBLIC HEALTH i ENVIROWNT <br /> AIR POLLUTION CONTROL DIVISION <br /> GENERAL INSTRUCTIONS FOR <br /> FUGITIVE PARTICULATE AIR POLLUTANT EMISSION NOTICE / CONTROL PLAN <br /> An Air Pollution Emission Notice and Fugitive Particulate Control Plan <br /> must be submitted with an application for an Emission Permit. <br /> Completion of this form will meet these requirements. Complete only <br /> those sections of this form which are applicable to your operation. All <br /> data given, such as production rates, topsoil removal, etc. , should be <br /> for that period of 12 months that is expected to have the greatest <br /> amount of activity, usually for the first year. Emissions will be <br /> calculated in terms of pounds per day and tons per year of particulates <br /> generated. Also, please submit the following, if available: <br /> 1. A map showing site location and boundaries, haul roads, and nearby <br /> residences or commercial structures. <br /> 2 . Production schedule. <br /> 3 . Schematic of the operation. <br /> 4 . Engineering or manufacturing data on control equipment. <br /> PERMITS REQUIRED <br /> Fugitive Particulate Emission Permits are issued for site-specific <br /> activities, such as mining, storage of materials, haul road activities, <br /> etc. <br /> The Division will use the information submitted on this form to estimate <br /> emissions from the activity. <br /> Any processing equipment, such as a crusher, screen deck, or asphalt <br /> plant is considered to be portable and requires a separate permit <br /> application, Air Pollution Emission Notice, and filing fee. <br /> This aids the applicant by requiring only a revised Air Pollution <br /> Emission Notice for relocation, whenever the equipment is moved to a new <br /> site. <br /> FEES <br /> A filing fee of $100 shall accompany each Air Pollution Emission Notice <br /> / Control Plan filed with a permit application. Permit processing fees <br /> will be charged for the amount of time the Division spends evaluating <br /> the permit application. These fees, (currently $50 per hour) plus any <br /> required public notice fees, must be paid before an Initial Approval <br /> Permit will be issued. Once the project has commenced operation, an <br /> inspection will be conducted by the Division. If all permit conditions <br /> are met, a Final Approval Permit will be recommended. Final Approval <br /> Permit processing fees must be paid before the Final Approval Permit is <br /> issued. <br />
