_ I. INTRODUCTION <br />• Western Slope Carbon, Inc. (WSC), in association with Foster-Miller <br />Associates Inc. and Radmark Engineering Inc., proposes a system capable of <br />disposing coal mine waste underground at its mine in western Colorado and <br />requests a negative determination from the Colorado Department of Health <br />with respect to a permit for subsurface disposal. <br />This preliminary application for a subsurface disposal permit addresses <br />items 1-9 on pages 6 and 7 of the 1 October 1977 Rules for Subsurface Disposal <br />Systems. Information pertaining to these items was requested by Mr. Michael <br />Liuzzi of the Colorado Department of Health in a letter to us dated 5 <br />September 1980. The location in this preliminary application of information <br />requested by the Department of Health is provided in Table I-1. <br />Material in this preliminary permit application was compiled and <br />developed by Beak Consultants Incorporated, Denver, Colorado, with the as- <br />sitance of Mr. Robert Rogers, P.E., geologist, of Cooper and Clark, also of <br />Denver. <br />Additional information on Hawk's Nest Mine can be obtained from the two <br />Western Slope Carbon Hawk's Nest Mine Interim Coal Mining Permit Applications <br />to Colorado Mines Land Reclamation Board Proposal Permanent State Program. <br />These two applications were: <br />1.
