If an authorisatioa under paragraph a) of <br />thin section i^ no longer accurate beeauee a different individual or <br />position has responsibility for the overall operation of the <br />lacility, a new authorisatioa satisfying the requirements of <br />paragraph a. of Chia section must be aubmittad to the Division, prior <br />to or toget)ur with any reports, information, or applications to De <br />signed by an authorised representative. <br />c) Grtilication. Any parson signing a document under paragraph a) of <br />thin section shall make the following certification <br />•I certify under penalty of law that thin document and all <br />attacheients were prepared under my direction or auparvision in <br />accordance with a system designed to assure that qualified personnel <br />properly gather and evaluate the information submitted. Haled on my <br />inquiry of the person or persona who manage the system, or those <br />persons directly rasponaible for gathering, the information submitted <br />is, to the beet o! my knowledge and belief, true, accurate, end <br />complete. I am aware that there are significant penalties for <br />submitting false information, including the possibility of tine and <br />imprisonment for knowing violations.' <br />• PART II <br />A. MANAGEMENT REQUIREMENTS <br />1. Chance in Diecharee <br />The permittee shall inform the Division (Permits end 8nforcement <br />Section) in writing of any intent to construct, install, or alter any <br />process, facility, or activity that le likely to result in a new or <br />altered discharge, and shall furnish the Division each plane and <br />specifications which the Division deems reasonably necessary to evaluate <br />the effect on the discharge and receiving etrasm. <br />The permittee shall submit this notice within two (2) weeks after making <br />a determination to perform the type of activity referred to in the <br />preceding paragraph. Process modiflcatione include, but aze not limited <br />to, the introduction of any new pollutant not previously identified in <br />the permit, or any other modifications which may result in a discharge <br />of a quantity or quality different from that which was evaluated in the <br />drafting of the permit including subsequent amendments. Following such <br />notice, the permittee shall ba required to submit a new CDPS application <br />and this application may be processed as sn application for an <br />individual permit if the application shows that the new discharge is <br />ineligible for coverage under the general pezmit.
