3.0 Field Inspection and Performance Monitoring Plan <br />MEI (or other qualified environmental personnel and mining operators) will conduct field <br />inspections of the azea during mining operations and various groundwater monitoring <br />activities. The inspection and monitoring requirements are summarized in Table 2, along <br />with various trigger criteria and response actions. The primary purpose of these <br />inspections will be to identify signs of environmental contamination and potential waste <br />streams, and to monitor fluid levels during dewatering activities for the potential <br />movement of free product into the gravel pit or sedimentation basin. A groundwater <br />sampling plan has also been developed to ensure that mining will not significantly <br />degrade existing groundwater quality on site, nor cause contamination to move offsite. <br />All samples will be collected, and handled, and submitted for laboratory analysis in <br />accordance with the site QA Plan (Appendix E). All field work will be conducted in <br />accordance with the standazd operating procedures contained in the QA Plan. Field <br />sampling equipment will be decontaminated prior to use at each location by washing with <br />a non-phosphate cleaner and double rinsing with distilled water. Decontamination liquids <br />and disposable field equipment (e.g. gloves, paper towels) will be containerized and <br />managed according to the Division s regulations. <br />3.1 Field Inspections <br />MEI will conduct and document weekly field inspections of the mining excavation, <br />sedimentation basin, and Trenches 1 and 2 to look for any signs of contamination. A field <br />inspection log will be completed for each site visit and a sample form is included as <br />Appendix B. Signs of contamination include evidence of historic landfill activities <br />(disturbed soils or visible solid wastes), stained soil or soil with hydrocarbon odor or <br />other unusual odor, and water with free product, hydrocazbon or other unusual odor, oily <br />sheen or unusual discoloration, <br />In the event any free product or product sheen is noted on any exposed groundwater in <br />the mining pit, in the sedimentation basin, in Trenches 1 or 2, or in the soils exposed by <br />the excavation, mining operations will cease immediately, and FMM will report orally <br />within 24 hours to the Division the circumstances encountered. The conditions will then <br />be evaluated and appropriate mitigation measures implemented. <br />The weekly field inspection shalt further note any change in the condition of any <br />stockpiled soils, solid waste or ACM, and any other pertinent observations. <br />Depending on field conditions and on the length of time mining will continue in a <br />specific area, the frequency of these inspections maybe reduced following the Divisions <br />approval. <br />On-site personnel (mining operators) will also be instructed to identify signs of <br />environmental contamination and potential waste streams on a more continuous basis <br />(see Section 3.6 for specific training requirements for on-site personnel).
