draw clean groundwater from south of the site north into the mining pit, with little <br />chance of movement of contaminated groundwater offsite. <br />Each Phase of mining will have a designated POC well that will be sampled for <br />groundwater quality periodically (Table 2). The POC well for the Phase 1 mining is <br />MW-9E, for Phase 2 a new well will be installed immediately downgradient of Subazea <br />3, and RW-6 will serve as the POC well for Phase 3 mining. The new POC well will be <br />installed in accordance with the protocols contained in the site QA Plan. The POC well <br />corresponding to the current Phase of mining will be sampled quarterly for one year and <br />then semiannually for the life of the mining Phase. POC wells will be analyzed for <br />BTEX and the protection levels will be set at the State Groundwater Standazds for BTEX, <br />in accordance with WQCC Regulation No. 41. These standazds are 5.0 ug/1 benzene, <br />1,000 ug/1 toluene, 700 ug/1 ethylbenezene and 10,000 ug/1 total xylenes. Exceedance of <br />a State Standard will trigger additional confirmatory sampling, followed by potential <br />adjustment of dewatering rates and/or remedial activity if exceedances aze verified. The <br />trigger criteria and response actions aze further detailed in Table 2. <br />3.4 Reporting <br />MEI will transmit via facsimile to the Division all fluid level measurements, as tabulated <br />on the Site Inspection Form (Appendix B), by 0800 of the morning following their <br />collection. This will allow the Division to assist in evaluating in a timely manner, any <br />effect of mining on groundwater conditions, especially during the first few months <br />following dewatering startup. In addition, the groundwater analytical results for sentinel <br />well, POC well, and Trench sampling will be transmitted to the Division within 24 hours <br />following MEI s receipt from the laboratory. <br />The weekly field inspection forms will be included in a brief quarterly report submitted to <br />the Division along with any soil or groundwater sampling results (exclusive of those <br />required under Section 3.5), fluid levels, waste disposition, and narrative describing the <br />general progress of mining, any environmental contamination encountered, the response <br />actions taken, and copies of attendance records for the tailgate training meetings etc. <br />Copies of all laboratory analytical reports will be included. These reports shall be <br />submitted within 30 days following the end of each quarter. <br />3.5 CDPS Monitoring and Reporting Requirements <br />Sepazate and additional monitoring and reporting requirements will be conducted in <br />accordance with Colorado Dischazge Permit System (CDPS) Permit No. CO-00446175 <br />issued for the site. A copy of this permit is included as Appendix D. <br />3.6 Training Requirements <br />Implementation of the field inspection portion of this WMCP relies heavily on mining <br />workers, who because they will be on site daily in the azea, aze the ones most likely to <br />first encounter and identify any signs of environmental contamination.
