Banta. <br />Division Director <br />July 15, 1991 <br />Mr. and Mrs. Howard Stone <br />8292 Teller No. 1 <br />Guffey, Colorado 80820 <br />Re: Stones Ranch Grav Pit, File No.: M-91-080, 110(2) Reclamation Permit <br />Application <br />Dear Mr. & Mrs. Stone: <br />Upon reviewing your application for a Mining & Reclamation Permit, received <br />July 3, 1991, we have determined that the following items must be submitted in <br />order for the application to be considered by the Mined Land Reclamation <br />Division: <br />Application Fee: Please submit an additional $750.00 for 110(2) <br />application fee (see enclosed fee schedule under HB 1115). <br />Plerase complete and return to the Division the enclosed page 2 and 3 of <br />they 110(2) application package. <br />EXFIIBIT J - Proof of Mailing Notices of Permit Application: You must <br />include proof of mailings of the "Notice of Permit Application" (forms <br />enclosed) sent to the board of county commissioners and board of <br />suFervisors of the local soil conservation district in the application. <br />We suggest you submit the date-stamped receipt for certified mailing or a <br />date-stamped copy of the notice form with signature acknowledging <br />personal service and receipt by the appropriate board. <br />Failure to follow the above will result in a decision to deny your <br />application. In accordance with Rule 1.5, a new decision date will be <br />established 30 days from the date of receipt of the application fee, page 2 <br />and 3 of the application package, and the notices. If you have already <br />initiated publication, you will need to republish notice which specifies a ten <br />(10> day public comment period from the new date of newspaper publication. <br />
