COLORADO DEPARTMENT OF PUBLIC HEALTHAND ENVIRONMENT, Water Quality Control Division <br />Rationale-Page B, Permit No. CO-0000132 <br />water quality impacts. A SWMP must be maintained for the Sanborn Creek Mine and Elk Creek Mine in accordance <br />with good engineering practices. (The plan need not be completed by a registered engineer.) The Division has a <br />guidance document available on preparing a SWMP. <br />The plan identifies potential sources ofpollution (including sediment) which may reasonably be expected to affect <br />the quality of stormwater discharges associated with mining or remediatian operations. In addition, the plan <br />describes practices which will be used to reduce the pollutants in stormwater discharges from the mining or <br />remediation operation, and to assure compliance with the terms and conditions of this permit. Facilities must <br />implement the provisions ojtheir SWMP as a condition ojthis permit. <br />There are separate sections in this permit addressing SWMP requirements for pollutant sources in active and <br />inactive porlions of the Sanborn Creek Mine and Elk Creek Mine. <br />1. SWMP For Active Mining Operations-Contents <br />a. Description ofmining activin'es . <br />b. Area subject to effluent limitations guidelines <br />c. Site map <br />d. Stormwater management controls <br />I) SWMP Administrator <br />2) Identification ofpotential pollutant sources and BMPs <br />3) Preventive maintenance <br />4) Good housekeeping <br />S) Spill prevention and response procedures <br />6) Employee education <br />7) Identification of discharges other than stormwater <br />e. Comprehensive inspection <br />2. SWMP For Inactive Mining Operations -Contents <br />a. Description ofmining activities <br />b. Site map <br />c. Stormwater management controls -Identification ofpotential pollutant sources and BMPs <br />I) Stormwater diversion <br />2) Sediment and erosion prevention <br />d. Comprehensive inspection <br />Additional SWMP Requirements apply ifthe Sanborn Creek Mine and Elk CreekMine seek terminations ofthe <br />stormwaterporton of this permit. In the event that thepermittee wishes to remediate an inactive mine site and <br />terminate permit coverage, prior to remediation act'vit'es, the SWMP must be amended to include the following <br />items, and thepermittee must notify the Division ofthe change in status. <br />e. Additionalltems <br />1) Documentation of land owner permission (if the permittee is not the owner) <br />1) Photographs <br />3) Description of remedial action to be performed <br />4) Description ojBMPs during remediation activities <br />S) Procedures for disposal of any water stored within mine waste <br />6) Description ofpermanent BMPs to be used, final site stabilizaton, and ultimate land use <br />SWMP Development. Coordination and Format <br />Some activities required under the SWMP may already be in place. However, the SWMP will require the permittee <br />to coordinate these activities with any necessary new activities in an orderly manner, so that the result is the <br />reduction or elimination ofpollutants reaching state waters from areas not limited by numeric effluent limitations. <br />It is the permittee's responsibility to notify the Colorado Division of Minerals and Gea[ogy (CDMG) of any changes <br />at theirsite resultingfrom the implementation of the SWMP. This notification allows the CDMG to review the <br />SWMP and incorporate any potential revisions which might be needed into the facility's reclamation permit. It is <br />important to verify that the site reclamation permit matches the SWMP. Such information is usually contained in <br />Exhibit G of 110 reclamation permits. <br />The SWMP may rely upon information developed for other similar pollution control programs such as the SPCC <br />(Spill Prevention Control and Countermeasures) plan, Voluntary CZeanup Plan, or the Mined Land Reclamation <br />Gan Revised: 4128/2004 <br />
