RESPONSE <br />• The following is a list of violations the applicant has received during the three-year period preceding the <br />application date of this permit revision. <br />Mine Identification - DMG Permit No. C 82-056 <br />MSHA ID No. -OS-03836; Issued 1983 <br />Violation Identification: CV -2001-004, March 22, 2001 <br />Issuine Reeulator~Authority: Division of Minerals & Geology <br />Brief Description of Violation: Failure to conduct water monitoring in accordance with approved plan. Failure to <br />maintain a ground water well and failure to submit surface water sampling data. <br />Date. Location and Tvoe of Administrative Proceedings_ A proposed assessment was sent to TCC on April 19, <br />2001 and proposed civil penalty of $1,000.00. TCC requested an assessment conference on April 24, 2001 and a <br />civil penalty of $ 300.00 was proposed based on the information presented during that conference.
