Burtis, Florence Pit, File No. M-2001-070, Receipt o[ 110(2) Hard Rock/Metal Mining <br />Reclamation Permit Application Package <br />Dear Mr. Gerity: <br />On July 18, 2001, the Division of Minerals and Geology received your the 110(2) Hard Rock/Metal Mining <br />Reclamation Permit Application package for the Florence Pit, File No. M-2001-070. Preliminary review of the <br />information received detemrined that the following items must be received before the Division can consider your <br />application as being submitted and technical review can begin: <br />APPLICATION FORM - Signature Paee: Notarization of Mr. Burtis' signature is improper. The <br />notary should complete the notarization as follows: <br />"The foregoing instrument was acknowledged before me this 13~ day of July 2001 , by <br />Wallace F. Burtis as Owner of Florence Pit ." <br />EXHIBIT J -Proof of Mailing Notices of Permit Auulication {Rule 6.3.10): Include proof of <br />mailings of the "Notice of Permit Application" sent to the Teller-Park Soil Conservation District Board <br />of Supervisors by submitting the date-starnped receipt for Certified Mailing (white receipt and green <br />Certified Mai] receipt) or adate-stamped copy of the notice form with signature aclmowledging <br />personal service and receipt by the Soil Conservation District. Also, refer to Rule 1.6.2(1)(a). <br />Teller-Park Soil Conservation District's address is: <br />Ms. Gale Glenn <br />Teller-Park SCD <br />112 S. A decision date will be established 30 days from the date of receipt of all of Ure requested <br />
