• <br />PART III <br />Page 13 of 16 <br />Permit No.CO-0037656 <br /> <br />A. SPECIAL REQUIREMENTS <br />The permittee shall submit a written request 90 days prior to discharge <br />to the Colorado Water Quality Control Division and the U.S. Environmental <br />Protection Agency for change in discharge points. The request shall be <br />accompanied by area maps which shall meet [he specifications set forth <br />below. Each discharge point shall be numbered consecutively (001, 002, <br />003, etc.) and designated on the map. The Division shall approve or <br />deny proposed discharge points in writing, or may in its discretion <br />require that [he permittee accept different effluent limitations as a <br />condition of approval or submit a new permit application. Approval of <br />additional discharge points shall not consist of permit modification, <br />and no public notice or hearing will be required. <br />2. The permittee shall submit a written request to the Colorado Water Quality <br />Control Division and the U.S. Environmental Protection Agency to deactivate <br />a discharge point. The Division will gran[ or deny [he request in writing. <br />If the request is granted, monitoring and reporting will not be required <br />for such points. Any discharge from deactivated points shall be considered <br />a violation of [he permit. <br />• <br />3• Area maps shall be made from USGS topographical maps (7.5 or 15-minute series) <br />or other appropriate sources as approved by the Colorado Water Quality Control <br />Division. Each revised area map shall be 8 I/2 inches by Il inches and <br />reproducible by rapid printing methods or as approved by the Division. Where <br />additional 8 I/2-inch by II-inch maps are required to show [he area of opera- <br />tion, thty shall be numbered and a key shall be shown on the first map. The <br />first map section shall have the company name, mine/job name, address, and <br />NPDES number clearly printed thereon. Also, one line of latitude and one <br />line of longitude shall be marked on each map section. The area map(s) shall <br />delineate the following, using the graphics as indicated: <br />(a) Existing Area of Operation (Solid Outline) <br />(b) Existing point source 0 (Solid Triangle} <br />(c) The projected area of operation for <br />the 5-year life of an NPDES permit ------~------(Dashed Outline) <br />(d) New point source discharge ~ (Open Triangle) <br />• <br />
