This report is filed in accordance with Rule 2.u2.b(3)(a) of the Regulations
of the Colorado i4ined Land Reclamation 3oard for Coal ~Nining, adopted in
accordance witn the Colorado Surface Coal Mining {kt, C.k.S. 34-33-1J1 et sec.
Tne purpose of tnis report is to update the Division on exploration ana
reclamation activities that nave occurretl in the pas[ 1311 days of active
prospecting. The report also indicates any cnanges in operations from wnat
was stated in the original notice of Intent sent to the Division. The
Operator :nay use tnis form, modify tnis form, or use a different format
altogether. ,
1. Attach a map that Snows:
a) 'rlhich proposed holes have been drilled to date,
b) Any actual or proposed relocations of holes,
c) The extent of all significant road construction completed to
date (exclude minor olade work), and
d) Current location of all drill rigs and test equipment.
