(e)(8) The inspection forms to be used to comply with this plan are shown as Attachment B. Ail above-ground <br />• tanks, piping, and valves will be visually inspected once a year. The inspection reports will be kept on file <br />for a least three years and will include at ]east three inspections. ]f problem areas aze found, afollow-up <br />inspection will be made after corrective measures are taken. ]nspections will be performed during the third <br />quarter of each year. <br />• <br />(e)(9)(i) Access to the mine site is controlled at all times. In addition, when the mine is not producing, all gates are <br />locked or guarded. <br />(e)(9)(ii) Valves pemvtting direct outward flow of tank contents are securely locked in the closed position when in <br />non-operating or non-standby status. <br />(e)(9)(iii) The starter controls on all oil pumps are locked in the "oft" position or located a[ a site accessible only to <br />authorized personnel when the pumps are in non-operating or non-standby status. <br />(e)(9)(iv) The loading/unloading connections of pipelines are securely capped or blank-flanged when not in service <br />or standby service for an extended period of time. <br />(e)(9)(v) Facility lighting is commensurate with the type and location of the facility. <br />(e)(10)(i) Personnel are properly instructed in the operation and maintenance of equipment to prevent the discharges <br />of oil and in applicable pollution control laws, roles and regulations. In addition, appropriate personnel will <br />receive training on the control of hazardous waste spills, handling procedures and appropriate emergency <br />response procedures.
