Mitchell: <br />The Division has reviewed your application and has the following adequacy <br />comments. <br />1, No seed mix, mulching applications or fertilizer types or rates <br />have been provided. <br />2. You have signed page 3 of the application form. Item (e) on this <br />page states: <br />"The mining operation will not adversely affect the stability <br />of any significant, valuable and permanent man-made structure(s) <br />located within two hundred (200) feet of the affected lands, <br />except where there is an agreement between the Applicant/Operator <br />and the persons having an interest in the structure that damage to <br />the structure is to be compensated for by the Applicant/Operator;" <br />Your application states that there is an Eastern Slope telephone <br />line and an IREA power line within 200 feet of the affected area. <br />You must provide us with agreements between you and the power <br />company and telephone company that damage to these structures are <br />to be compensated for by you (the Applicant/Operator), <br />
