The applicant must provide <br />the DMG with an affidavit or receipt demonstrating that the change or addition was filed with the County <br />Clerk and Recorder no later than the dose of business on the day the change was filed with the DMG. <br />Please acknowledge that you have received a copy of the following: <br />- 1-page cover letter to Gregg Squire, DMG, dated June 17, 2002, from Lynn Mayer, L.G.
