Cedar Drvivon Director <br />Akron, CO 80720 <br />Re: Akron Ready Mix, File No. M-97-0I5, Receipt of 112 Construction Materials Reclamation <br />Permit Application Package <br />Dear Mr. Darrell: <br />On February 28, 1997, the Division of Minerals and Geology received your application fora 112 Reclamation <br />Permit. Preliminary review of the information received determined that five (5) copies of the following items <br />must be received before the Division can consider your application as being submitted and technical review <br />can begin: <br />APPLICATION FORM: Based on information contained in the application received you must <br />submit a Construction Material Regular 112 Operation Reclamation Permit application form and <br />exhibits. The application received by the Division has not been used since July, 1995. Please find <br />enclosed a new application package for your use and to be submitted to the Division along with any <br />missing exhibits or information required. You must submit the original ivory application to the <br />Division along with 4 copies of the application form. <br />EXHIBIT C -Pre-minine and Minine Plan Maas) of Affected Lands to include the location of <br />any recorded easements (Rule ti.4.3): One or more maps may be necessary to legibly portray the <br />following information: <br />Show the owner's name of any easements and location of these easements on the area of <br />affected land and within two hundred (200) feet of the affected land. <br />EXHIBIT N -Source of Leval Right to Enter to include holders of any recorded easements (Rule <br />6.4.14): You must provide the source of your legal right to enter and initiate a mining operation on <br />the affected land (to include agreements with holders of any recorded easements). <br />EXHIBIT O -Owner(s) of Record of Affected Land (Surface Area) and Ownets of Substance <br />to be Mined to include holders of anv recorded easements (Rule 6.4.15): Please submit a <br />complete list of all owners of record of the affected land and owners of the substance to be mined <br />(include holders of any recorded easements). <br />
