On October 18, 2001, the Division of Minerals and Geology received your 112 Construction Materials
Reclamation Permit Application package for the Fort Lyon Canal Company Pit, File No. M-2001-097.
Preliminary review of the information received determined that the following items must be received before the
Division can consider your application as being submitted and technical review can begin:
APPLICATION FORM: Page 5 and 6, Responsibilities as a Penuittee: The Division requires that
these be initialed by a wrporate officer. Page 7, Signature Page: The Division requires that this page
be signed by a corporate officer. The Division requires that the corporate seal be affixed to this page.
F.XIIIBIT C -Pre-minin¢ and Minim Plan Maols) of Affected Lands (Rule 6.4.3): One or more
maps may be necessary to legibly portray the following information:
(a) all immediately adjacent surface owners of record;
(b) the name and location of all creeks, roads, buildings, oil and gas wells and lines, and
power and communication lines on the area of affected land and within two hundred (200) feet of all
boundaries of such azea;
(c) the existing topography of the azea with colour lines of sufficiem detail to portray the
direction and rate of slope of the affected ]and;
(d) the total area to be involved in the operation, including the area to be mined and the
area of affected lands (see defuution of "Affected Land");
(e) the type of preset vegetation covering the affected lands; and
(f) in conjunction with Exhibit G -Water hrformation, Subsection 6.4.7, if required by the
Office, further water resources information will be presented on a map in this section.
