Walther <br /> Ezecurive Director <br />Manuel Torrez <br />The Fort Lyon Canal Company Div lion Director <br />750 Eient Ave. <br />Las Animas, CO 81054 <br />Re: Fort Lyou Canat Company Pit, File No, M-2001-097, Receipt of 112 Construction Materials <br />Reclamation Permit Application Package <br />Deaz Mr. Torrez: <br />On October 18, 2001, the Division of Minerals and Geology received your 112 Construction Materials <br />Reclamation Permit Application package for the Fort Lyon Canal Company Pit, File No. M-2001-097. <br />Preliminary review of the information received determined that the following items must be received before the <br />Division can consider your application as being submitted and technical review can begin: <br />APPLICATION FORM: Page 5 and 6, Responsibilities as a Penuittee: The Division requires that <br />these be initialed by a wrporate officer. Page 7, Signature Page: The Division requires that this page <br />be signed by a corporate officer. The Division requires that the corporate seal be affixed to this page. <br />F.XIIIBIT C -Pre-minin¢ and Minim Plan Maols) of Affected Lands (Rule 6.4.3): One or more <br />maps may be necessary to legibly portray the following information: <br />(a) all immediately adjacent surface owners of record; <br />(b) the name and location of all creeks, roads, buildings, oil and gas wells and lines, and <br />power and communication lines on the area of affected land and within two hundred (200) feet of all <br />boundaries of such azea; <br />(c) the existing topography of the azea with colour lines of sufficiem detail to portray the <br />direction and rate of slope of the affected ]and; <br />(d) the total area to be involved in the operation, including the area to be mined and the <br />area of affected lands (see defuution of "Affected Land"); <br />(e) the type of preset vegetation covering the affected lands; and <br />(f) in conjunction with Exhibit G -Water hrformation, Subsection 6.4.7, if required by the <br />Office, further water resources information will be presented on a map in this section. <br />
