Fluid levels will be measured by MEI or other qualified personnel using a <br />. device capable of detecting water and products to an accuracy of 0.01 feet {e.g. using an <br />electronic interface probe). Fluid levels will be recorded on the Site Inspection Form <br />(Appendix B). If product is detected in any wells not previously containing product, or if <br />product levels/thickness changes significantly in MW-2 or WP-3, or if significant <br />drawdown is observed in any of the wells, then pumping will cease until the data can be <br />evaluated (in consultation with the Division) and appropriate mitigation measures <br />implemented. <br />3.3 Reporting <br />The weekly field inspection logs will be included in a brief quarterly report submitted to <br />the Division along with any sampling results (exclusive of those required under Section <br />3.4), fluid levels, waste disposition, and narrative describing the genera] progess of <br />mining, any environmental contamination encountered, the response actions taken, and <br />copies of attendance records for the "tailgate" training meetings etc. These reports shall <br />be submitted within 30 days following the end of each quarter. <br />3.4 CDPS Monitoring and Reporting Requirements. <br />Separate and additional monitoring and reporting requirements will be conducted in <br />accordance with Colorado Discharge Permit System (CDPS) Permit No. CO-00446175 <br />issued for the site. A copy of this permit is included as Appendix D. <br />• 3.5 Training Requirements. <br />Implementation of the field inspection portion of this WMCP relies heavily on mining <br />workers, who because they will be on site daily in the area, are the ones most likely to <br />first encounter and identify any signs of environmental contamination. Therefore, all on- <br />site personnel will be required to attend a "tailgate" meeting held with MEI at the mining <br />site prior to their initial commencement of work. During the meeting, MEI will review <br />the critical elements of this WMCP, instruct the miners on how to recognize <br />environmental contamination in the field, and describe the appropriate response actions to <br />betaken. Each worker will sign the form included in Appendix B documenting that they <br />attended the tailgate training prior to the commencement of work at the site. A copy of <br />the WMCP will be retained at the mining site at all times, for reference and direct access <br />to pertinent telephone numbers and contact information. A summary of information to be <br />given in the tailgate meetings is shown in Appendix C. <br />4.0 Waste Characterization. <br />MEI will perform all field characterization duties. MEI`s Standard Quality Assurance <br />/Quality Control Plan ("QA Plan") outlines proposed sampling protocols (see Appendix <br />E}. In the event waste is encountered, MEI will determine if the waste is hazardous, based <br />on the following definition: <br />
