A®MIPIISTR~iT9®1~163itV® ENF®FZCEAflEF9T <br />3.01 Permid ~ipplication Requirements and Procedures <br /> <br />A Prior to commencing construction, installatioq expansioq alteration or repair of an <br />individual sewage disposal system, a written application shall be submitted to the local <br />health department and a permit shall be issued. The application shall provide the <br />following information: <br />1. Name, address and telephone number of the owner of the property. <br />2. Name, address and telephone number of the owner's agent, if applicable. Any <br />person acting as agent for the property owner shall submit written <br />authorization from the owner giving that person permission to serve as agent <br />in acquiring the permit. <br />3. Name of system contractor (installer). <br />4. Legal description of the property. <br />5. Letter of approval from County Land Use Department, in cases involving <br />systems for mobile homes or existing limited-use or seasonal-use structures <br />which are to be upgraded or converted to year-round use. <br />6. Complete site plan including an alternate site for a leachfield in case of a <br />failure. <br />7. Accurate directions to property. <br />8. Type of building use. <br />9. Type of water supply and location of proposed and existing water lutes. <br />10. Results and supporting data of soil percolation or hydraulic conductivity tests, <br />if applicable, conducted by an Registered Professional Engineer at the location <br />of the proposed system. <br />11. Proximal location of bedrock at proposed location of system. <br />12. Proximal location of the groundwater table at the proposed location of system <br />11 <br />:~ <br />
