FOREST SERVICE EVALUATION OF PLAN OF OPERATIONS <br />Continuance of FS-2800-5, Plan of Operations for Mining Activities on NFS lands <br />A. Required Changes/modifications/special mitigation for plan of operations: <br />In Reference to the proposed Plan of Operation for Glacier Peak submitted by Joseph Dorris on November <br />10, 2005. <br />I. GENERAL INFOILVIATION <br />E. Expected duration of operation is five years from the signature date of the proposed Plan of Operation <br />by the authorized officer. <br />G. Expected date for completion of reclamation will be November 30, 2011. <br />IV. DESCRIPTION OF OPERATIONS <br />A. Access: <br />FS Clarification/Revision: <br />• New temporary road access should be in accordance with the Decision Notice signed by Brent Botts on <br />June 7, 2006. <br />• The operator is required to request permission from private property owners prior to utilizing a private <br />access road, constructing any temporary access roads or mining oa private property. Proof of written <br />consent should be provided to the Forest Service and State and Local agencies prior to use. <br />• The operator may request aRight-of--Way permit to access closed system roads in order to access his <br />mining claim. <br />• The operator must use necessary precautions to not damage any utility lines or associated facilities en <br />route to dig sites in the project area. <br />• Equipment maybe walked-in off system roads. If tracks or road development becomes evident an <br />additional bond maybe required for continued operation. <br />• No system road maintenance is authorized without approval of the Forest Service authorized officer. <br />C. Project Description. <br />FS Clarificafion/Revision: <br />Timber: <br />• All trees over 8" in diameter must be designated for removal prior to excavation of dig sites and must <br />receive Forest Service approval. <br />• Fallen trees must be limbed, lopped and scattered. Slash will be no deeper than 12" on the ground. <br />Bole material lazger than 4" in diameter will be removed to location where the public can access it for <br />firewood. In lieu of cutting, trees maybe pushed or excavated with mechanized equipment. All <br />resulting stumps must be buried in the adjacent dig site when reclamafion occurs. <br />• The operator would be required to purchase a firewood permit to use any cut trees that aze not directly <br />incident to mining operations. <br />Fire: <br />• All internal combustion engines including but not limited to generators, vehicles, mechanized mining <br />equipment and power tools, must be fitted with an approved spark arresting device or muffler in good <br />working order. <br />• The operator must have and maintain hand tools for fire suppression in the event of a wildland fire. <br />
