FROM GLV8Fasoc. • PHONE N0. 970 565 100• Nov. 13 1997 03:07PM PS
Ms. Lemay
November 12, 1997
Page Four
- Absence of Statement of Application for Local Permits. Certain essential
permits seem to be missing from the Applicant. In his Reclamation
application, Mr. Stone needs to supply proof of a permit for the following:
w
- StnrmwatPr - from the Colorado Department of Health and
Environment -Water Quality Control Division.
- F~,<inmen F,micc;onc P rrn;t -from the Colorado Department of
Health and Environment -Air Pollution Control Division.
- G1RE iccionc P i 8s it Po 1 ~tion miccionc No i APFN -from
the Colorado Department of Health and Environment -Air Pollution
Control Division.
- Road AR rR oaf -Montezuma County Roads Department and the
Bureau of Land Management.
- No ; . of lntPnt i(NC)il -from the Colorado Division of Minerals and
Geology.
- _F.n 'ronm n al Acc ccm n or Environmental impact Sta m nt -
from the Bureau of Land Management.

- .ro end Wa Pr -from the Office of the State Engineer, Division of
Water Resources of the Department of Natural Resources.
- Berm Places. There is no stated plan to create berms or barriers that serve
as a visual/noise barrier and soil storage. For similar gravel pit operations,
berms are required to be at least 20 feet high and covered with topsoil.
Topsoil provision should be imposed so as to require revegetation and
m;n;mal disturbance of the berm. If this Application is granted, we request
that such berms be required, maintained and inspected.
- Vegetation Information. The current application's description of the
affected vegetation is incomplete. Described as "sparse," this area is
actually well covered, although there are signs that non-native plants, such
as lmapweed and Russian thistle, were introduced to the area due to a prior
gravel pit excavation just west of the proposed new quarry. There is no plan
