Watcher <br /> Executive Director <br /> Michael fi Long <br /> Division Director <br />Re: H & H Stone Quarry # 3, FDe No. M-2000-154, Receipt of 112 Construction Materials Reclamation Permit <br />Application Package <br />Dear Mr. Holley: <br />On December 8, 2000, the Division of Minerals and Geology received your 112 Construction Materials Reclamation Permit <br />Application package for the H & H Stone Quarry IY 3, File No. M-2000-154,. Preliminary review of the information received <br />determined that the following items must be received before the Division can consider your application as being submitted and <br />technical review can begin: <br />APPLICATION FORM: Page 7, Signature Page: The Division requires that the operator nattre and the notarized <br />name be the same (i.e. H & H Stone Co., Inc.). <br />EXHIBIT N -Source of Legal Ridrt to Enter (Rule 6.4.141: You must provide the source of your legal right to <br />enter and wliate a mining operation on the affected land. This may be a Dopy of access lease, deed, abstract of title or <br />a current tax receipt. A statement by the landowner and acknowledged by a Notary Public stating that the <br />Operator/Applicant has legal right to enter and mine is also acceptable, <br />EXHIBIT R -Proof of Filing with County Clerk and Recorder (Rule 6.4.181: You must submit an affidavit or <br />receipt indicating the date on which the application was placed with the local County Clerk and Recorder. <br />Your application will not be considered submitted until the information listed above is received and found sufficient to begin <br />our review. A decision date will be established 90 days (tom the date of receipt of all of Ore requested information. <br />Additionally, if you have already published notice you will need [o republish notice, after the Division considers the application <br />submitted. 71tis notice must be published once a week for four (4) consecutive weeks, starting within ten (10) days of the date <br />your application is considered submitted. We will notify you when you should iniOate republication of your noOce. The firm <br />date for receiving comments is the 20th day after the fourth publication or the nest regular business day. <br />This letter shall not be construed to mean that there are no other technical deficiencies in your application.
