Preliminary review of the information <br />received determined that the following items must be received before the Division can considec your application as being <br />submitted and technical review can begin: <br />APPLTCATTnN FORM, 5ignahue Page (Page 7): On page 1, item 1 of the applicaflon shows that the <br />ApplicanUoperator name is Oldcasde SW Group, Inc. dba United Companies of Mesa County. On page ~ the <br />ApplicanUoperator name is shown as being Oldcasde SW Group, Inc. Please be advised that the Applicam/operator name <br />must always be consistent throughout the application form Therefore, the signature page must be redone. <br />F.XAiRiT C .Permanent Man-Made Ctntch.rec (Role 6_d.1Q~; You mast provide information sufficient to <br />demonstrate that the stability of any structures located within two hundred (200) feet of the operation or affected land <br />will not be adversely affected. You must correct Exhibit S, Pages S-1 and the compensation agreements with all of the <br />owners of permanent man-made structures so that there is a wmplete list of owners and an compensation agreement for <br />each structure owner. <br />Your appliation will not be considered submitted until the information listed above is received and found sufficient to begin our <br />review. A decision date wi11 be established 90 days from the date of receipt of all of the requested information. Additionally, if <br />you have already published notice you will need to republish notice, aRer the Division considers the application submitted. This <br />notice must be published once a week for four (4) consecutive weeks, starting within ten (10) days of the date your application is <br />considered submitted. We will notify you when you should initiate republication of your notice. The final date for receiving <br />comments is the 20th day aRer the fourth publication or the next regular business day. <br />You Lave sixty (60) days from the above date to submit all necessary documents that the Office needs for an application to be <br />considered filed. If, at the end of the sixty day period, the application has not been determined to be filed with the Office, the <br />Off ce shall deny the application and terminate the appticaflon file. <br />This letter shall not be construed to mean that there are no other techaical deficiencies in your application.
