Please be advised that <br />there are new permit application requirements based on rule changes which <br />became effective January 1, 1991. Please be advised that during our initail <br />review we determined that the following information must be submitted for <br />review by the Division: <br />Enclosed please find an approved 110(2) permit application form. Please <br />conplete this form and have an authorized representative of Lincoln <br />County sign and have their signature notarized. Return one original <br />signature form and a cony to the Division. <br />EXHIBIT E - Reclamation Plan Map: <br />1, Indicate the gradient of all reclaimed slopes (horizontal:vertical) <br />sufficient to describe the post mine topography; <br />2. Indicate where vegetation will not be established and the general <br />area(s) for shrub or tree planting; <br />3. If ponds are a part of the reclamation plan, outline the final shore <br />configuration of the ponds and shallow areas if the future land use <br />is for wildlife: <br />4, State the average thickness of replaced overburden; and, <br />5. State the average thickness of replaced topsoil, <br />EXHIBIT I - Proof of filing with County Clerk: You must provide an <br />affidavit or receipt indicating the date on which the application was <br />placed wiht the local county clerk and recorder. <br />
