Upon reviev~ing your application for a Mining & Reclamation Permit/Amendment,
dated October 9 , 1985 , the staff has determined that it is
deficient income ete un er t e olorado Mined Land Reclamation Act, Article 32
of Title 34, C.R.S. 1973, as amended (1976 Session Laws, House Bill 1065),
The following items must be submitted in order for the application to be
considered as filed (pursuant to the MLRB definition of "filed", as found in
Rule 1.1(10) of the Rules and Re ulations of the Mined Land Reclamation Board,
as amended, effective July, 1978
1. Please submit 2 copies of the enclosed aoDlication form cmm~lati=ly fi11Pd
out and signed.
2. Please provide a map which portrays the site permit boundaries (i.e. scale
not larger than 1" _ X00' nor smaller than 1" = 660') This map should show the
_existing permit boundary and the proposed new boundary. Please include roads,
buildings, utilities, streams, etc. located within 200' of the permit boundary.
