Please submit 2 copies of the enclosed aoDlication form cmm~lati=ly fi11Pd <br />out and signed. <br />2. Please provide a map which portrays the site permit boundaries (i.e. scale <br />not larger than 1" _ X00' nor smaller than 1" = 660') This map should show the <br />_existing permit boundary and the proposed new boundary.
