COLORADO DEPARTMENT OF PUBLIC HEALTH & ENVIRONMENT <br />• <br />AIR POLLUTION CONTROL DIVISION <br />GENERAL INSTRUCTIONS FOR <br />FUGITIVE PARTICULATE AIR POLLUTANT EMISSION NOTICE /CONTROL PLAN <br />An Air Pollution Emission Notice and Fugitive Particulate Control Plan must be submitted with an application for <br />an Emission Permit. Completion of this form will meet these requirements. Complete only those sections of this <br />form which are applicable to your operation. All data given, such as production rates, topsoil removal, etc., should <br />be for that period of 12 months that is expected to have the greatest amount of activity, usually for the first year. <br />Emissions will be calculated in terms ofpounds per day and tons per year ofparticulates generated. Also, please <br />submit the following, if available: <br />A map showing site location and boundaries, haul roads, and nearby residences or commercial structures. <br />Production schedule. <br />3. Schematic of the operation. <br />Engineering or manufacturing data on control equipment. <br />Fugitive Particulate Emission Permits are issued for site-specific activities, such as mining, storage of materials, haul <br />road activities, etc. <br />The Division will use the information submitted on this form to estimate emissions from the activity. <br />Anyprocessing equipment, such as a crusher, screen deck, or asphalt plant is considered to be portable and requires <br />a separate permit application, Air Pollution Emission Notice, and filing fee. <br />This aids the applicant by requiring only a revised Air Pollution Emission Notice for relocation, whenever the <br />equipment is moved to a new site. <br />FEES <br />A filing fee of $100 shall accompany each Air Pollution Emission Notice /Control Plan filed with a permit <br />application. Permit processing fees will be charged for the amount of time the Division spends evaluating the permit <br />application. These fees, (currently $50 per hour) plus any required public notice fees, must be paid before an Initial <br />Approval Permit will be issued. Once the prof ect has commenced operation, an inspection will be conducted by the <br />Division. If all permit conditions are met, a Final Approval Permit will be recommended. Final Approval Permit <br />~ocessing fees must be paid before the Final Approval Permit is issued. <br />I:\CPUFORMS.98A\FD APEN.93A <br />
