The recommended decision date for yow application is currently scheduled for November <br />15, 2005. Two (2) copies of your adequacy response must be submitted and received by the Division by <br />no later than 5:00 P.M. on October 31, 2005 to allow Division staff sufficient review time. Please be <br />advised that you will need to provide all infomtaticn or commitments requested below and/or resolve all <br />remaining concerns of the Division on or before the Novcmber 15, 2005 recommendation date. Failure to <br />do so will result in a Division recommendation of denial of yow application. Please note that Highway <br />119 Access Points "A", "B", & "C" referred to below pertain to "A" -original permit application filed on <br />November 9, 2004, "B" -amended application filed on April 11, 2005, and "C" -revised final location <br />filed on August 19, 2005. <br />A. Public Notice Requirements -The Applicant will need to confirm that all public notice requirements <br />per Rules 1.6.2, 1.6.5, & 1.6.6 have been completed. This will include the Applicant verifying it has sent <br />written notice to the Silver Dollar Metro District and any other property/utilitylsttvcture owners within 200 <br />feet of proposed access points "B" or "C" that were not properly noticed during the public notice period <br />for the amended application. Those individuals or utilitylstructure owners that will be receiving a notice <br />from the Applicant will have 42 days from the date of receipt of such notice to submit written comments to <br />the Division. The notice(s) must be sent in a timely manner to those owners so that the 42 day time period <br />for written comments by those owners ends no later than November 7, 2005; so that the Division will have <br />sufficient time to review those written commnents prior to the November 15, 2005 Division recommended <br />decision data. As a part of this requirement, the Applicant will need to submit certified mail talon receipts <br />from each individual or utility/structure owner noticed. Please provide. <br />B. Blasting Plan - Alan Sorenson is reviewing specific Best Management Practices (SNP's) to be <br />implemented prior to, during, and after blasting operations to minimize the generation of dust from <br />blasting operations.
