The following procedures must be <br />adhered to and are considered conditions of that Emission Permit: <br />(1) The applicant must inform the air Pollution Control Division tr.irty (30) <br />days prior to the start-up of the stationary source covered by the <br />enclosed Emission Permit. Ia the case of portable sources fifteen (15) <br />days' notice is required. Please comp lece the reverse side of this form <br />and mail to the air Pollution Control Division by the time 11mit specified <br />above. (Reference: air Pollution Control Regulation No. 3, Section J. 1.) <br />{Z) Within thirty (30) days after start-up of the equipment covered by sn <br />Emission Permit, an on-site inspection gill be made by personnel from the <br />Air Pollution Control Division or its delegateu representative. This <br />inspection is to determine if the terns and conditiors of the Permit have <br />been pLOperly satisfied. <br />~) if as emissions source test(s) is required as a condition of the enclosed <br />Emission Permit, this test(s) mus- be conducted according to methods <br />approved by the Division before final approval can be issued. <br />(4) The enclosed Emission Permit is revocable under the conditions prescribed <br />by air Pollution Control Cormlission Regulation No. 3. <br />• ~~~~~ <br />Signature <br />
