THIS PERMIT is issued by the Mined Land Reclamation Board, Department of Natural Resources,
State of Colorado.
RECITALS
A. The Pertnittee, H & H Stone Co., Inc. desires to caldud a muting operation lmown as H & H
Stone Quarry # 3, for the purpose of extracting stone. The PertniGee will nd mule any secardary commodities.
B. On May 14, 2001 the Mined Land Reclamation Board (the "Board") approved the Pertnittee's
appbcation for this permit, fixed the amount of the financial wartanty and directed that this permit be issued upon
the filurg with the Division of Minerals and Geology (the "Divisiar") of performance warranty and financial
warranty (or warranties) m the amount so fixed m form and substance approved by the Division. Said warranties
have been filed with the Division.
C. If the PemtiVee desires to extras materials other than those listed ur (A), a separate permit may
be required.
D. On May 14, 2001 the Board made the following findings:
1. The application far this permit complies with the requirements of the Colorado Mined
Land Reclamation Ad forthe Extraction of ConsWdion Materials, C.RS. 34-32.5-101 d sue., as amended, and
with all applicable local, state and federal laws;
2. The operator will nd adversely affect the stability of any significant. valuable, and
permanerrt man-made swdure located within two hundred feet of the Aflectsd Land, except where there is an
agreement between the Operator and the perscals having an urterest m the structure that damage to the structure is
to be compensated for by the Pemtittce; and;
