This operation will be in Fremont County. It
was previously mined under permit and subsequently abandoned by another operator. The
original file for this operation is probably in the possession of your Abandoned Mined Land
Reclamation program.
There are five copies of the application. One copy is labeled on the envelope as containing
"Original Signatures." That copy contains forms with original blue ink signatures. The other four
contain copies of those documents.
Included is a check (#00050205) in the amount of$2,325.00, the application fee for a new quarry
permit.
Please contact myself if you have any questions or desire to discuss any aspects of the plan or
permit materials.
Respectfully,
Mark A.
