OTHER PERMITS AND LICENSES REQUIRED EXHIBIT M <br />The following permits are needed for the site in addition to the DMG permit: <br />1) County (special use) permit - a full special use application is being supplied to the Mesa County <br />Planning office for approval. <br />2) A COE Permit is needed since minor wetlands and waters of the United States. The COE is <br />evaluating the data to see if a national or individual permit is needed. <br />3) An NPDES combined process water/storm water permit will be needed for the site since the pit <br />dewatering will result in continuous discharge. <br />4) A Gravel Well permit is needed from the Division of Water Resources since water will be <br />encountered in the extraction and some will be used for consumptive use during the operation and <br />evaporation will also be considerable after the lake is created. <br />5) A fugitive emissions permit is needed for the extraction and processing since the permitted <br />production will exceed 80,000 tons per year. The portable crusher/screen plant, the concrete batch <br />plant and the portable asphalt plant will have their own emissions permits. <br />6) A spill and containtrtent plan will be made since fuel storage on site will occur. <br />7) No stormwater management plan is needed since the site will contain the 100 year 24 hour <br />runoff. <br />8) A State Highway Access Permit is needed for vehicles entering Highway 6/S0. <br />15 Road Pit 10/02 47 <br />
